Uniting us towards a healthier Frederick County

None of us can affect measurable change on our own.

Our Coalition serves our community by providing a structure for passionate leaders to collaborate so that together, we can make a lasting impact and improve the lifelong wellbeing of Frederick County residents.

Current Priorities

These priorities were selected through a robust assessment process with substantial community input within the Community Health Needs Assessment (CHNA) process. Now, health partners collaborate with members of our community in one of our three Health Improvement Planning Workgroups and turn ideas into action.

Our Partners

In order to improve the lifelong health of all Frederick County residents, we involve the two largest organizations that impact our community’s health: the Frederick County Health Department, and our health care system Frederick Health. To reinforce this commitment, our by-laws require that we host a representative from each organization as an active board member, and we’re grateful for their support and participation.

Beyond these two founding organizations, we actively recruit partners from all around the county, taking care to represent the wide diversity within our community. Health is complex, and the factors influencing our health vary greatly. To appropriately improve the health and wellbeing of our county, we must learn from the diversity of our population and seek to understand the diverse effects on our health.

We need stories like yours.

We all have our own health journey, and we witness the health and wellbeing of our families and loved ones. At the Coalition, we seek to make a lasting impact on our community’s health, and that success relies on our capacity to understand the challenges and opportunities that our residents face.
